I think this is very possible. Have you noticed any inconsistency in his delivery, either from the stretch, or the windup?
Not really, it all looks pretty consistent. Like I said I think the runners are a bi-product of the problem, not the cause of it.

Interesting but 3 times today jake went to the windup with the bases loaded. That seems to tell me that jake is thinking about his mechanics from the stretch. He had thrown about 30 pitches when trouble set in, today. Hard to see fatigue there.
Anyway, I might send him down to AA to work on his mechanics. Can you send a guy to extended spring if he is not hurt.?

You can pitch from the stretch with the bases loaded because runners have nowhere to steal (unless they are going to triple steal which doesn't happen...like ever) A ton of guys do it, and coaches I've met teach guys to go back to the windup with them loaded. Think about it, you stretch to be able to pick off/hold runners, but would you risk a pick off move when an errant throw (much easier to do than you'd think) would lead to at least one run scored and runners on 2nd/3rd?
He's not overthinking things and not delivering because of it, if he were, his stuff would be suffering from it. The only change in his stuff at all is his command of the fastball, the secondaries are the same, and they would be all over the place too if it were him thinking too much.
He gave up his first run on a sac fly in the 3rd, that was well after 30 pitches. His mechanics are not the problem, it's his command, and it's not batter to batter, it's inning to inning, which is why he gets lit up like he does because he's not just making a mistake, his stuff is completely off for a while so he gets lit up.

Disappointing loss.
I was at the game, really disappointing being there. I've seen plenty of worse losses, don't get me wrong. But what was so bad about it was the Dodgers pitcher, who I had never heard of looked like he was going to get shelled. 2 of 3 isn't bad, just wish it wasn't a game I went to that happened to be the loss.
